For the request URL try putting the php file for the board you want and in the name and value section add a name com and a value for a comment, you can just use the default (ｷﾀ━━━(ﾟ∀ﾟ)━━━!!) and add a name upfile with the value of either @file.png or file.png I'm not sure which would work.
